A double block and bleed arrangement may fail because either block valve passes, the bleed path is restricted or the test boundary is wrong. Bleed pressure and flow indicate where to investigate, but they do not prove the leaking seat. Verify the line-up and test each block valve under approved conditions.

What a failed arrangement tells you
Double block and bleed isolation uses two closed boundaries with a vented space between them. The bleed provides evidence about pressure entering or remaining in that space.
A pressure rise or continuing flow at the bleed suggests leakage or trapped pressure. It does not identify the failed block without further checks.
Heating, a restricted bleed, wrong line-up or another connection can distort the result. Treat the arrangement as unproven until the test basis is checked.

Verify the test boundary first
Preserve the initial readings before moving any valve. Record upstream pressure, downstream pressure, bleed condition, valve positions and instrument status.
- Confirm both block valves and the intended flow direction.
- Check that connected branches and bypasses match the test line-up.
- Verify the bleed path is open, clear and routed safely.
- Compare local valve travel with remote position indication.
- Check whether pressure and temperature are stable enough for testing.
Pressure at the bleed is screening evidence. Confirming evidence comes from the approved isolation or seat test applied to each boundary.

Use bleed behaviour to plan the next test
Bleed behaviour can narrow the investigation when the boundary and instruments are sound. Keep the cause list open while working through the pressure directions.
- Pressure returns after venting. One block may pass, or another connected path may refill the cavity.
- Flow continues at the bleed. A block may leak under the present differential, but the source side still needs confirmation.
- No bleed flow appears. The cavity may be depressurised, or the bleed path may be blocked.
- Pressure changes with process direction. The active differential may help select the next approved seat test.
Common causes include debris, scored seats, dried material and incomplete closure. Wrong stops, drive faults, blocked bleed passages and instrument error can also explain the result.
Do not label one block as failed only because it sits on the higher-pressure side. Seat design and cavity behaviour can change how pressure appears.
Test each block as its own boundary
Where the procedure allows, test the first block against a known pressure condition. Then test the second block using a condition that makes its result clear.
Keep the method, hold condition and acceptance basis recorded. A lower bleed rate after movement suggests change, but only an approved retest can confirm acceptance.
Incomplete travel needs mechanical diagnosis before seat treatment. Use the mid-travel fault guide when position evidence does not agree.
Where separate testing cannot be done safely, do not infer the culprit. Maintain the restriction and escalate for an engineered isolation and test plan.
Restore function through clear gates
Cleaner, cycling or sealant may help limited fouling or seat leakage. These actions are not suitable for every valve or service.
Before treatment, confirm four gates. Injection fittings must be suitable and in good condition. The valve design and service must suit injection.
OEM guidance applies, and operations must permit the work. Use controlled amounts within all valve, fitting and equipment ratings.
Watch pressure response, uptake and process condition. Stop for rapid pressure rise, fitting leakage, no uptake, rising torque or unexpected flow.
After treatment, repeat the approved test for both blocks and the bleed space. Record each result separately, including any valve that remains restricted.
If either boundary still fails, double block and bleed protection remains unavailable. Arrange planned repair, replacement or another approved isolation method.

Does flow from the bleed prove that one block valve passes?
It suggests pressure is entering the cavity, but it does not identify the leaking block by itself. Confirm the line-up, bleed condition, pressure direction and each valve with an approved test.
Can both block valves pass at the same time?
Yes. Both valves may leak, or one may leak while the other has incomplete travel or damaged seats. Test each boundary rather than assigning the fault from one pressure reading.
Can sealant injection restore double block and bleed isolation?
Sometimes, for suitable valves with limited seat leakage. The fittings must be in good condition, the design and service must suit injection, OEM guidance applies and operations must permit treatment.
What if the leaking valve cannot be identified safely?
Keep the isolation unproven and escalate for a proper engineering assessment. Use an approved alternative isolation plan before work that depends on double block and bleed protection.
