2. Record the starting condition before treatment
Describe the operating state, line condition and permission for movement. Record what first suggested the fault.
Pressure rise, continuing flow, noise and high torque are screening signs. They do not confirm seat damage, passing or the source of resistance.
Name the confirming test, its boundary, conditions and result. Where confirmation is not possible, keep the diagnosis open.

3. Log the suitability gates and work controls

Repeat the approved leakage or isolation test under recorded conditions. Keep the acceptance basis clear and comparable with the starting result.
Use honest outcomes: accepted, improved but not accepted, unchanged, or stopped. Add the reason and evidence for the outcome.
If the result remains uncertain, assign assessment rather than hiding it in notes. Severe damage may need isolation, internal inspection, repair or replacement.
6. Turn records into maintenance decisions
Compare results for the same valve across visits. Trends in leakage, torque, uptake and fitting condition can change its priority.
